Question. My wife and I indulge in lots of foreplay before intercourse. But at the first moment of arousal, she gets a throbbing pain in the vagina. It persists for three to five minutes and is sometimes unbearable. Please tell us what the cause would be and suggest a remedy for the same. Thanks.

Answer. There is a natural spasm which occurs when a woman reaches orgasm, which is what she is feeling at present. Ask her to bear it or take an antispasmodic tablet before starting. Hopefully it will help.
